Adding a consistent form to the sidebar

I am looking to add a consistent form to the sidebar that will appear on every page. I've found a template that I've been able to use called "Form Builder" to create the form, but I can't figure out how to get that widget to appear on every page. Anybody have any advice/thoughts?

Hi Jason, If they give you HTML, you can simply enter it into a text widget.
