Adding Plain Page Template for Gravity Forms

So I am currently trying to add a Gravity Form to WHP3 in a Fancy Box modal window, however, when I try to add the form, I can not properly create a plain page template so that only the form displays, as seen below... any help would be appreciated! What issues are you running into when creating the new page template? Are you the following the step correctly in the tutorial and the form on the page doesn't display or work properly?

Yeah, when I try to display the form the modal appears, however, it simply shows

	    * Edit My Profile
	    * Dashboard
	    * Log Out
	
	# Edit Page
	# Add New
	
	    * Post
	    * Page
	
	# Comments
	# Appearance
	
	    * Widgets
	    * Menus

Thats after I create the "plain page" template which is described in the tutorial.

Nevermind, I had to adjust a few jQuery bits and its starting to work, now to only make those bits above disappear! :)
