Subscription Refund?

I had no idea I still had a pagelines subscription, thought I'd cancelled it ages ago, so was surprised to be charged $99 this month. I didn't receive any warning emails about the upcoming renewal, or I would have unsubscribed before the charge.

How do I request a refund?
