Activating PL5 theme failed

Hi,
 
I might need some support activating the PL5 theme. Footer, header and wp dashboard are gone after activation and entire website background turns black.
I have several PL5 sites and this is the only one that causing the problem, so possibly nothing wrong with PL5

ya its an error of some kind, id first investigate plugin conflicts and possibly an issue with the way you're installing or the host

check the logs on your server, maybe will give you a clue. Look around for an error

Maybe this is causing the problem? @Andrew Would appreciate any help with solving this.  Thanks

In addition to the above here is what I'm getting in WP now after enabling WP_DEBUG:

Thanks @Andrew This is now resolved. 

If anyone come across similar issue, in my case WP wasn't updating my PL5 plugin for some reason.
