WP Toolbar Rendering a Link that Cannot Be Removed

I'm at my wits end trying to find a way to discourage a link from appearing on the WP Toolbar (the one that appears on the front end).  When a user is logged in and facing the front end of the site, there is a link that appears that is not part of the typical Wordpress meta, etc junk.  It's this: Mode: Type | Template: Default.  It does not appear on the back end of my site nor does it appear on any of my other sites that don't employ DMS.  I inactivated all of my plugins and the cursed little rodent remains.  Any ideas how I can suppress this?  I need to leverage the WP toolbar for clients that are logged into their portal pages and this link is utterly out of place.  Any help would be greatly appreciated.  Thanks
