Which form plugin works best with DMS? I downloaded Poppy but do not see a widget. I need to revise the contact form to include more information for a Request Estimate form.

 

The WordPress Contact Form Maker  does not seem to work with Pagelines. It shows up put will not size for the sizebar and will not show up when I view the site. Can anyone help me with a good free form plugin that works with Pagelines and has capcha. Thanks

 

If Poppy is the answer what is the best way to included it as a widget and can I revise the fields? Thanks
