Can you explain to me how to use the captcha as in the example?

 

Have installed a captcha plugin, but it does not show up.

 

What am I doing wrong?

It does not use any captcha plugin...

You simply provide a question and expected answer like in the example 'whats 2 + 5?'

And where do I set that question? Do I need to go in the class files? And which one? Which method can I alter to do so?

in my 3.5.1 I cannot find the settings screen you mention, under which tab is this? The screenshot could be more helpful if it would show the left side more.
